Consolidated ward SOE for 2018-19

Ward Health and Sanitation Committee is an important field level institution catering to local public health needs. National Health Mission provides a grant of Rs. 10000 to each ward in the district to carry out various public health activities. In this context, it is requested to submit a consolidated statement of receipts and expenditure for each ward. Medical Officers in charge/Superintendents are directed to verify, sign and submit this consolidated statement of expenditure by 29th May.

Ward Health & Sanitation Committee funds shall be transferred to institutions only on submission of this consolidated statement of expenditure. Use the format in the link below for consolidation and submission of expenditures. Medical Officers must verify the opening balance as on April 1, 2019, fund receipt from NHM in 2018-19 (2 tranches of fund release - one in April and another soon after floods), total expenditure in 2018-19, and fund recouped to NHM and fund balance as on March 31, 2019. Fund recouped to NHM should not be shown as expenditure.
